Description
Alta View Hospital is hosting a free 6 week class series on “Living Well With Chronic Conditions” and is taught by one of our nurses and the curriculum is evidence based from Stanford.
We are recruiting community members suffering from 1 or multiple chronic conditions that can benefit from self-management tools such as diabetes, chronic pain, arthritis, fibromyalgia, hypertension, depression & COPD. Refreshments and incentives at each class.
